INVITATION TO TENDER

The University of Botswana invites 100% citizen-owned, locally registered companies to submit proposals for the below-named tender.

SPECIFIC REQUIREMENTS:

(a)  Local companies should be registered with the Public Procurement and Regulatory Authority in the following categories:

Code: 202 Medical Supplies and Related Equipment

Sub code(s): 04: Medical Consumables, Healthcare aids, surgical and orthopaedic and laboratory reagents.

PPRA registration is subject to online registration verification

 (b) Valid Tax Clearance Certificate number and Tax Identification Number (TIN) or exemption thereof, both certified and issued by BURS. Such solidity of the Tax Clearance is subject to online registration verification.

(c)  Duly completed Declaration Form for Tendering Purposes (T2.2 GM)

(d)  Duly completed and signed Form of Offer and Acceptance (T2.2GA)

(e)  Duly completed Certificate for Authority of Signatory

(f)  Certificate of incorporation with company extracts showing details of directors and shareholders and Beneficial Owners

(g)  Certified copies of identity documents for shareholders and beneficial owners and directors

(h)  Duly completed Integrity Agreement Form

1. Letter of Support from the main Distributor or Manufacturer

(j)  Proof of payment for the tender document

Bidders who fail to submit the above-mentioned documents (as listed in Circulars 1 and 5 of 2016) will be requested during the evaluation to submit them within 2-5 days of notification. Bidders will be notified through a read-receipt email as an alert. Non-responsiveness by the bidder shall result in disqualification of the bid.  

NB All Bidders shall submit the completed and signed Form of Offer and Acceptance (T2.2GA) in the first instance without fail. Otherwise, the bid shall be disqualified.

In order to receive the RFP document, interested companies must follow the below steps:

1. Click on the payment button shown at the end of this advert. (Nonrefundable tender  fee is P500.00 (VAT inclusive).
2. Follow the on-screen steps to complete payment for the tender.
3. The tender document will be automatically sent to your email once payment has been approved.

NB: If for any reason during the tender period it becomes necessary to vary the Tender Documents, an Addendum will be published on the website https://www.ub.bw/connect/tenders as and when such amendments are made. It is the sole responsibility of the bidders who have purchased the tender document from the website to keep themselves abreast of such amendments by continuously checking the website before submitting the tender document.
