SPECIFIC REQUIREMENTS:

The Tenderer shall furnish the following documentary evidence to demonstrate that it meets the compliance criteria:

  1. Citizen companies should be registered with the Public Procurement and Regulatory Authority in the following categories:

Code: 103 collection and disposal of Hazardous Material

    Sub code(s): 03 Clinical waste collection, Disposal and related services. PPRA registration is subject to online registration verification

 

(ii) Valid Tax Clearance Certificate number and Tax Identification Number (TIN) or exemption thereof issued by BURS. Such validity of the Tax Clearance is subject to online registration verification.

(iii) Declaration Form for Tendering Purposes  (T2.2 GM)

(iv) Completed and signed form of Offer and Acceptance (T2.2GA)

(v) Certificate of Incorporation with Company extracts showing Company Directors and Share holders. Subject to online registration verification

  1. Permits for waste disposal sites (attach valid waste disposal permit). Provide permits for waste disposal sites, landfill site/agreements with licensed waste disposal sites and/or landfill site/incineration site. (bidders that subcontract this service must attach valid agreements with subcontractors and an affidavit – oath commissioner). 

    7. Insurance coverage in force, workers' compensation and public liability insurance, or a letter of intent to provide workers compensation                    and public liability insurance with limits of cover upon the award of a tender. Successful bidders shall be required to submit the insurance                  covers prior to signing of the contract. 

 

  1. In case of joint ventures and associations between 100% citizen owned firms, a legally binding intent to the joint venture/association agreement, stating the lead company in such joint venture, must be submitted. And fully complete the attached joint venture form (if applicable). 

     9. Name of the person and contact details: telephone and email address to be contacted in case of any request for clarity on the submitted bid. 

 

  1. Attach proof of payment for the purchase of this tender 

 

In order to receive the RFP document, interested companies must follow the below steps;

  1. Click on the payment button shown at the end of this advert. (Nonrefundable tender  fee is P500.00 (Vat Inclusive), Youth to 50% (P250.00)
